研究興趣

本研究室的研究以轉譯醫學為主,可歸納為下列五個領域:
1. 癌症的硼中子捕獲治療  (Boron-neutron capture therapy, BNCT) 自2010年起,台北榮總癌症中心及核醫部跨部門合作,積極從事以非侵入式硼中子捕獲治療方式,來治療臨床頭頸癌復發的患者,至今累計超過三十個以上的病例,是除美日以外,世界上少數擁有此治療技術的國家之一。本研究室很榮幸參與此群體計畫,並負責建立腦瘤動物模式、含硼藥物ADME、最適當投藥和BNCT照射時間以及療效的基礎研究。
2. Adoptive cell transfer (ACT)的免疫療法在癌症免疫治療之應用  本研究室將實驗動物的T細胞自小鼠胰臟分離出來,先以流式細胞儀篩選出CD4+和CD8+表現較佳的T細胞,轉殖報導基因(pGBeLT, granzyme B as the promoter, enhanced luciferase and Tomato fluorescent protein as reporters)後,在體外大量培養。進行癌症免疫治療前,先給予小鼠一劑低劑量的化療藥物,例如: doxorubicin或paclitaxel,然後再輸入轉殖有報導基因的T細胞進行免疫治療。研究結果發現,只需要原來2/5 數目的T細胞,即可有效抑制腫瘤的生長。此研究成果大大改善臨床T細胞免疫治療法,常面臨T細胞數不足,導致腫瘤控制不理想的情況。
3. Psycology and Psyco-oncology  精神醫學及精神腫瘤醫學的研究 99mTc-TRODAT-1 是臨床診斷帕金森氏症的核醫藥物。台灣是世界上第一個將此藥物推到臨床使用,並被全球各國所接受,而使用於臨床的國家。本研究室負責小動物實驗的診斷效果及安全測試。這方面的研究成果,被美國國家衛生院NIH 收錄到Molecular Imaging & Contrast Agent Database (MICAD)的資料庫內。本研究室再進一步負責利用[*I]ADAM診斷血清素(serotonin)神經系統相關疾病的小動物臨床前研究,例如: 甲基安非他命、古柯鹼和K他命等毒品對血清素神經系統的影響;以及憂鬱症治療前後的療效評估。2012年本研究室領先全球,發表mirtazapine (樂活優)抗憂鬱症藥物,可經由血清素神經系統及免疫系統,有效抑制腫瘤的生長。
4. 類風濕關節炎及骨質疏鬆症之研究   類風溼關節炎(rheumatoid arthritis, RA)與過度活化的免疫細胞有關,並且在患者的關節中發現其釋放出的細胞激素有吸引更多免疫細胞來攻擊自身關節滑液膜、軟骨和硬骨的能力,而造成難以治癒的免 疫性疾病。目前臨床用於治療RA的藥 物,大多為類固醇(steroid)、非類固醇的抗發炎藥物(non-steroid anti-inflammation drugs)以及一些針對細胞激素的生物製劑,然而這類的治療藥物不僅價格昂貴,也會產生相當程度的副作用。本研究室以RAW 264.7巨噬細胞和DBA/1J小鼠模式,分別羽扇豆醇醋酸(lupeol acetate, LA), liposoaml LA以及不同濃度LA和薑黃素組合(combination),對於類風濕性關節炎以及骨質疏鬆症的療效。除量測各種相關激素、免疫細胞、發炎因子外,關節炎的判讀使用臨床關節炎積分表,並以正子斷層造影([18F]FDG/micro-PET)、及以micro-CT測量骨質密度等評估療效。
5. 以游離輻射結合肝癌標靶藥物蕾莎瓦(sorafenib)找出治療人類肝癌、結腸癌和口腔癌的最佳組合模式及機制  肝癌數十年來一直排名是世界十大癌症第五名,也高居國人癌症罹患率的第三名。截至目前為止,臨床上一直缺乏有效的治療方式。即使應用蕾莎瓦肝癌標靶藥物,平均也只讓病人多存活三個月。因此尋求更有效的肝癌治療策略,一直是臨床上亟需的。本研究室合併游離輻射和蕾莎瓦於負有三種人類肝癌動物模式之治療。研究結果發現,合併治療較任一單一治療,有更佳的腫瘤控制率,而且先以蕾莎瓦治療數天後,再給予放射治療,相較兩者同時給予,或者先給予放射治療,之後再給予蕾莎瓦治療,有顯著更好的肝癌控制率,老鼠的存活天數也顯著增長。這種合併治療模式,是經由抑制ERK/NF-kB的訊號路徑,來抑制腫瘤的生長,同時可以降低放射治療過程,癌細胞產生大量的NF-kB,導致其下游effector蛋白質(anti-apoptosis, angiogenesis, cell proliferation, invasion etc.)的過度表現。我們也發現,以此方式治療口腔癌,可有效抑制癌細胞侵蝕齒顎骨。

       About 50 graduate students obtained master degrees (MS) under Dr. Hwang’s supervision, in which Ya-Fang Chang (2006) and Tzu-Chun Chen (2013) obtained the In, Shin-Ro Outstanding Research Award (Only three out of all graduate students of the year in campus are eligible to be awarded each year). Wei-Tien Tai (2007) and Yu-Chan Chen (2014) obtained the In, Shin-Ro Excellent Research Award (Only six out of all graduate students of the year in campus are eligible to be awarded each year). 轉譯醫學領域的研究

在陽明服務二十七年來,在轉譯醫學的研究成果,主要可歸納為下列五個領域如下:
 
1.癌症的硼中子捕獲治療 (Boron-neutron capture therapy, BNCT)
    自2010年起,台北榮總癌症中心及核醫部跨部門合作,積極從事以非侵入式硼中子捕獲治療方式,來治療臨床頭頸癌復發的患者,至今累計超過三十個以上的病例,是除美日以外,世界上少數擁有此治療技術的國家之一。本研究室很榮幸參與此群體計畫,並負責建立腦瘤動物模式、含硼藥物ADME、最適當投藥和BNCT照射時間以及療效的基礎研究。本研究室在此領域的研究成果發表在下列論文:

2.Adoptive cell transfer (ACT)的免疫療法在癌症免疫治療之應用
    本研究室將實驗動物的T細胞自小鼠胰臟分離出來,先以流式細胞儀篩選出CD4+和CD8+表現較佳的T細胞,轉殖報導基因(pGBeLT, granzyme B as the promoter, enhanced luciferase and Tomato fluorescent protein as reporters)後,在體外大量培養。進行癌症免疫治療前,先給予小鼠一劑低劑量的化療藥物,例如: doxorubicin或paclitaxel,然後再輸入轉殖有報導基因的T細胞進行免疫治療。研究結果發現,只需要原來2/5 數目的T細胞,即可有效抑制腫瘤的生長。此研究成果大大改善臨床T細胞免疫治療法,常面臨T細胞數不足,導致腫瘤控制不理想的情況。本研究室在此領域的研究成果發表在下列論文及Book chapter:

3.Psycology and Psyco-oncology 精神醫學及精神腫瘤醫學的研究
    99mTc-TRODAT-1 是臨床診斷帕金森氏症的核醫藥物。台灣是世界上第一個將此藥物推到臨床使用,並被全球各國所接受,而使用於臨床的國家。本研究室負責小動物實驗的診斷效果及安全測試。這方面的研究成果,被美國國家衛生院NIH 收錄到Molecular Imaging & Contrast Agent Database (MICAD)的資料庫內。本研究室再進一步負責利用[*I]ADAM診斷血清素(serotonin)神經系統相關疾病的小動物臨床前研究,例如: 甲基安非他命、古柯鹼和K他命等毒品對血清素神經系統的影響;以及憂鬱症治療前後的療效評估。
    2012年本研究室領先全球,發表mirtazapine (樂活優)抗憂鬱症藥物,可經由血清素神經系統及免疫系統,有效抑制腫瘤的生長。本研究室在此領域的研究成果發表在下列論文及Book chapters:

4.類風濕關節炎及骨質疏鬆症之研究
    類風溼關節炎(rheumatoid arthritis, RA)與過度活化的免疫細胞有關,並且在患者的關節中發現其釋放出的細胞激素有吸引更多免疫細胞來攻擊自身關節滑液膜、軟骨和硬骨的能力,而造成難以治癒的免疫性疾病。目前臨床用於治療RA的藥物,大多為類固醇(steroid)、非類固醇的抗發炎藥物(non-steroid anti-inflammation drugs)以及一些針對細胞激素的生物製劑,然而這類的治療藥物不僅價格昂貴,也會產生相當程度的副作用。本研究室以RAW 264.7巨噬細胞和DBA/1J小鼠模式,分別羽扇豆醇醋酸(lupeol acetate, LA), liposoaml LA以及不同濃度LA和薑黃素組合(combination),對於類風濕性關節炎以及骨質疏鬆症的療效。除量測各種相關激素、免疫細胞、發炎因子外,關節炎的判讀使用臨床關節炎積分表,並以正子斷層造影([18F]FDG/micro-PET)、及以micro-CT測量骨質密度等評估療效。本研究室在此領域的研究成果發表在下列論文,並已經取得兩項中華民國發明專利,美國發明專利也同步申請中:

5.以游離輻射結合肝癌標靶藥物蕾莎瓦(sorafenib)找出治療人類肝癌、結腸癌和口腔癌的最佳組合模式及機制
    肝癌數十年來一直排名是世界十大癌症第五名,也高居國人癌症罹患率的第三名。截至目前為止,臨床上一直缺乏有效的治療方式。即使應用蕾莎瓦肝癌標靶藥物,平均也只讓病人多存活三個月。因此尋求更有效的肝癌治療策略,一直是臨床上亟需的。本研究室合併游離輻射和蕾莎瓦於負有三種人類肝癌動物模式之治療。研究結果發現,合併治療較任一單一治療,有更佳的腫瘤控制率,而且先以蕾莎瓦治療數天後,再給予放射治療,相較兩者同時給予,或者先給予放射治療,之後再給予蕾莎瓦治療,有顯著更好的肝癌控制率,老鼠的存活天數也顯著增長。這種合併治療模式,是經由抑制ERK/NF-kB的訊號路徑,來抑制腫瘤的生長,同時可以降低放射治療過程,癌細胞產生大量的NF-kB,導致其下游effector蛋白質(anti-apoptosis, angiogenesis, cell proliferation, invasion etc.)的過度表現。我們也發現,以此方式治療口腔癌,可有效抑制癌細胞侵蝕齒顎骨。本研究室在此領域的研究成果發表在下列論文:
