研究興趣

  治療用超音波(Therapeutic Ultrasound)目前已成為一個新興潛力的研究領域,其原理是利用非侵入方式將高強度聚焦超音波(High Intensity Focused Ultrasound)能量導入所需治療之區域,利用組織對機械波能量之吸收造成溫昇,將該區域之溫度提昇到43℃~45℃之間或利用高強度機械波壓力產生空穴化作用,產生高溫及高壓對組織進行破壞等現象來去除或治療腫瘤。此外,前人的研究指出聚焦式超音波結合超音波顯影劑能以非侵入方式將特定區域的血腦屏障暫時開啟。本實驗室研究是探討超音波顯影劑濃度與超音波聲壓力對局部開啟血腦屏障後治療效果與藥物動力學分析。由Evans blue在腦組織的滲出量得知,在相同的聲壓力下高濃度的顯影劑可造成較高的Evans blue的滲出量,而當顯影劑濃度相同時,較高的聲壓力也可以得到類似的結果。此外,在高濃度的顯影劑下,可以使血腦屏障的開啟較集中於焦點區域。從組織切片觀察得知,在適當的低濃度顯影劑時,可以在不傷害正常腦組織的情況下將血腦屏障開啟,然而在高濃度時會造成細胞凋亡的現象而其凋亡數量隨著距離焦點位置的增加而遞減。此研究驗證適當的顯影劑濃度能在不傷害正常腦組織的情況下增強穿顱式超音波將血腦屏障開啟的效率並且更集中於焦點區域。此種超音波藥物目標區域傳輸系統的原理是超音波的機械波與打入病患血管內的微氣泡相互作用所產生的空穴化效應,促使在超音波施打區域的血管通透性增加,達到只有在病灶區域的藥物濃度提昇,進一步提高治療效果。同時,因為能降低病患治療藥物的施打劑量,相對減少了藥物引發其他副作用的可能性。研究開發以磁振造影導引暨監控聚焦超音波開啟血腦障壁與藥物釋放系統,以超音波將治療藥物導引至局部的治療目標區域並增強病灶區域的藥物傳輸量,提高治療效果與治癒率並且以核醫影像做藥物動力學分析。目前本實驗室已開發的「超音波藥物傳輸監測裝置」技術,適用於各種組織器官。其原理是以都卜勒超音波量測脈衝聚焦式超音波施打血管位置前後之血流速度,除了可以確認施打位置的正確性外,更可由計算而得之收縮期峰值血流速度(peak systolic velocity, PSV) 和搏動指數(pulsatility index, PI)來間接即時監測超音波施打目標區域的藥物滲透量。此關鍵技術及監控軟體可相容於現有各大廠牌超音波系統,未來市場規模潛力極大。另外,將超音波熱療結合放射治療系統之開發亦為本實驗室之研究方向。
